i've encountered the mentioned duplicate bug as well, but not as often.

it happened first on a very big file, so restarting helped to free memory (sh*tty Mac OSX).
Last week it happens on a new small project, so i believe it's not a memory issue at all [:((]

i'll try to nail it down.
